Not sure about the heart attack, but Tim Belknap was diabetic and he is believed to have introduced insulin use into bodybuilding.

Being a true Sergio fan and Arnold admirer, I really believe Mentzer's solo shots are the most impressive, amazing and shapely legs, beautiful vacuum, complete set of arms, even amazing square chest (fdb pose)
In reality he structure was way smaller than Arnold and he was no where near size of Sergio, but he gave illusion of "greater" size.

Yes, Menzter on his own is a very credible, complete and fantastic physique. That big rib cage, vacuum, well proportioned and with good size top to bottom and front to back. Very complete. I'd say he could have had more size on his chest in the side chest, though, but it's nitpicking.

Amazing rare picture, shows how large Arnie's frame was.
Standing alone Arnold had standout chest and arms, his sloping delts and widish waist gave illusion of not so broad shoulders.
Only Tinerino and Ferrigno could match his massive frame.
Even Sergio Oliva wasn't structurally so broad next to him.

That was the 1975 IFBB Mr. America. Robby won, Roger Callard was 2nd and Mentzer third. Mentzer won it the next year.
