He's had spinal fusion (back surgery). It actually shortens the spine (he is noticeably shorter nowadays) and the result is that the internal organs get scrunched up (less space for them). Roddy Piper has the same problem. He went from 6'1.5" to 5'9" (spinal surgery) and because of his upper torso being shortened, it looks like he has a gut.
