Why painful ?
Are we talking about Lee Priest? I can't see the pic, but we all know he used to get ridiculously big in the offseason. Duchaine claimed he was Lee walking around at 270+, full of water but not too fat.
If true, that's a sick amount of weight for someone as short as he to carry around. I know in my first year of college, I stupidly bulked from about 215-220 up to over 250 at 5'6.5". I certainly had trouble getting around...I was never out of breath walking around campus, but after walking from the parking lot to some of my classes (maybe 1-1.5 miles?), I had horrible shin splints, my feet felt like someone was driving nails into my soles, and my calves, which are a genetic strength, would pump up to the point I literally thought my skin would tear.
As I said, the weight gain was stupid, and had it been more gradual, maybe I would have acclimated. But I'm glad I didn't, because I learned a lot from the experience: naturals who have trained hard for over four years can't just pile on weight and expect much of any of it to be lean tissue; there's more to life than being big, especially as a fat fuck; you might impress some dudes in the weight room, but who fucking cares about that: college age honeys certainly don't; it's damned embarrassing to saunter into class and, surreptitiously as you can, slip out of your shoes to get some pain relief :/
