After all those health problems he's still injecting oil.

He probably took a huuuge debt (relatively speaking) to start up his business. I really doubt he has ever written down some numbers, a basic balance sheet, etc to infer what he can expect from this business.

Is he the backer of the business or just running it?

Any way you slice it, starting a business is EXPENSIVE. Nobody with medical debt that can't pay the hospital would be starting a business with capital they don't have. I'm skeptical it's his money in it... I would be leaning more towards he's just part of it or runs it and likely has equity or draws a salary.
