I didn't wait for the video to load, but if this is the video of the fellow falling backward from in front of a red squat rack, he had massive injuries.

His left tibia broke, and needed a titanium rod repair. His right femur broke in two areas, his petellar tendon ruptured, quad tore,  and there -was some damage to his LCL ligament.

  What causes that? Don't say 685 lbs.  :)  I mean he never started the squat. The knee just gives like that?

Everyone that was there said his knee just went out from under him. To my eye, it looks like he wasn't comfortable with the squat, but could'nt find a way to ditch the bar.

The fellow trains at IronSport Gym, perhaps someone here trains there.

Funny you point that out, though I dont see the relation between the two posts. The irony here imo is the fact in itself that you're pointing that out  ;)

he didn't start the squat but he did unrack the weight and had to balance it, so that 685 was being supported by all of the above mentioned bodyparts.
