I have two.
The first one was in a private gym where you had to stack the plates on the dumbells and use the butterfly clips to keep them in place. I saw a guy doing pullovers when the weights slid right off and smacked him square in the face. It was brutal.
The other was a guy doing close grips rows on a low pulley with some heavy weights when the cable snapped. This guy smacked himself in the face with the double grip as he flew backwards doing a flip right onto the hard floor. It was funny as shit.

Not much of an accident cause the dude didnt get hurt, but this wigger dude who was about 5'7 140 with a hat turned sideways and wifebeater on put 505 on the squat rack and was doing 1/16th reps. He eventually dropped the bar and the sound when it hit the safety bars was deafening. The manager came over and said if he did that again he'd be gone.

ive seen a guy doing leg presses and he kept locking his knees out and then he started doing these one leg at a time, locking his knees out, and his leg snapped back so his leg was bent the wrong way at the knee. sucks to be him.

Both Best & Worst Ever:
"Trainer" (ie. 180lbs, 6'1", skinny kinda guy) was training some schmoe kid (probably 16-17) and had him doing squats on a swiss ball.
Loses balance, goes face first into the dumbbell rack. Loses minimum two teeth, and a MASSIVE pool of blood. I'm a nurse and attempted to try and take care of him, and the trainer comes over (as he was busy talking to some chick) and goes "what did you do?" to me!
After the ambulance took the kid away, I brought him up to the office (as I had worked at the gym before) and showed him the footage, and said 'hope you have insurance'.
Look on his face was priceless

When I was 15 and first started to lift in a dump of a gym that was under the local watering hole, I saw my brother dump 495 off his back and on to a spotters foot. It started to roll off his traps and he yelled for his spotters to dispatch but the doofus just stood their. He didnt try to help or to dodge the bar. He ended up with 2 broken toes and and a broken foot.

my nephew was all proud he got one of those piece of shit weider benches with a leg ext attachment on it. he was doing leg ext when he pinched his finger in that piece of crap it started to bleed and he screamed and jumped around like the time he was a kid and turtles 3 came out for nintendo, only this time he was in pain not ectasy.{gotta love those turtles} he actually broke two fingers. nice safety testing mr joe.

About 10yrs ago at the gold's gym in Sunrise, Me and My training partner took some bong hits before training legs. Go through 135, 225 and 315. Everything was going good until we put 405 on the bar. My boy gets under the bar starts dropping his ass then he just fell over forward, We were in the power rack and thankfully someone left the catch bars in there, if not that would have decapitated him for sure.

Saw a guy at a powerlifting meet break his forearm benching this weekend. Snapped it. Heard he already had the surgery and can't wait to get back to training!

Saw and older guy 45-50 he looked like one of those guys that make a new years resolution and goes to the gym for 2 weeks. Anyways he loaded 2 plates on a preacher curl machine no warm up or nothing and tried to curl it. I heard a weird snapping sound then he made a pretty good shriek and dropped the weight, he just ran straight outta the gym didnt change or nothing. I think he tore a bicep.

Hockey Player; Going dumbbells bench press on a swissball. Well you know how they do that with 20pds or 30 pds, for balance and shit for Hockey; Well this fool was like 6,1 and 210 and had 70lbs dumbbells on his hands doing a bench press on the swissball when is 'POPPED' and his elbow hit the hard floor and the weight of 70lbs snapped one of his wrist back the wrong way and first: Screamed, got up screamed again and then started cursing and then ran out of the gym yelling 'black this black that' Two points First his Hockey career is over and second is he isn't white.

I saw a kid bottom out with squats with 315 and he just ran to the locker room. don't know what happened. I figured I was done spotting him, so I left.

I saw a kid get stuck at the bottom of the hacksquat machine... Too trainers came over and couldn't lift it up because the kid was so far down he couldn't even help himself with the trainers taking off about 200 or so lbs pushing up. So they just threw off the plates one by one. The kid was alright but was also a bit embarrassed

I was there when this happened. I know you have all seen this video (and mocked it). The guy ended up breaking a femur, a tibia and tore up both knees. After 9 weeks he is just getting out of the hospital.
I also saw a girl in my gym try to remove a 45lb plate off a bar on the incline for her boyfriend and it slipped through her hand landing squarely on top of her foot. Weeks later she showed me the x-ray and the bones in her foot looked like a jigsaw puzzle.

Resolutionist story. Lady comes in the gym for hte first time since highschool according to her. First workout with a trainer not 15 minutes into it, she blacks out and plants her fact on the corner of an icarius legpress machine. Nasty gash but kept her grill. :(

I was moving a bench, I dropped the bastard on my big toe and smashed it open. Got my toenail cut off and some stitches. >:(

I enjoy seeing the occasional bench press/ should press mishap where the person doesnt use the 'collar/clasp' and the wieght starts to slowly slip on one side and after it falls down, the seesaw effect happens and the other side begins to unravel. I stop what I do to applaude it every time hahahahah

The only thing is that it was in his car when his foot was on the gas pedal.

I was doing over head cable tricep extension with a v grip bar and it slipped out of my hands and hit me in the forehead. It made me see white for a second then red!! It split my forehead open. I also had 225 roll out of my hands on bench presses. A bent bar and an open grip don't mix sometimes.

Every so often you see this.
Some monster, usually a 150 lb teenager loads up the bench press or decline or incline press.
They start off ok, hammer out 2 or 3 agonizing reps. 0.1 second negatives (i.e. just dropping it on their chests) and then the eternal battle to get it back to the top.
It always amazes me how they can keep going, but, eventually gravity catches up to them, the look of fear on their faces when they realize there is no way they can rack the weight is worth the gym membership every time.
2 weeks ago some idiot had the weight crash on his head on decline, he took off and never has been back since.

a guy in my gym 4 years ago doing incline bench with 185, he was struggling on a rep and a member was rushing over to him and said the guy muttered 'i got it' so he backed off. he heard a horrific crash and turned around to see the guy screaming with both hands over his face with blood gushing. he had crash landed the bar right on the bridge of his nose and it went all the way through to the skull. he was air lifted from the gym parking lot to hospital in b'ham 60 miles away. he had about 8 recovery operations.

Saw a dude last week running too fast on a treadmil, tripped and flew about 4 feet in the air, landed head first on the treadmill then was violently throw back off the treadmill. Funniest thing I've ever seen. Major tread burn, ripped his clothes up pretty good and the humiliation was probably too much for him. ;)

Back about 8 years ago, there was this guy ( I liked to call him Jim Gymnasium) that was your typical toolbag bodybuilder wannabee...You know the type...He goes over to the cable crossover, and moves the pin to the bottom of one of the stacks.Instead of triceps pushdowns, I guess he's gonna do triceps extensions, where you grab the bar on the high pulley and face away from the machine, bending at the waist. Anyway, this clown thinks he's about to do the whole stack, but little does he know that he didn't shove the pin in all the way, so there is no resistance on the pulley.. He gives a twisting tug on the bar to get into position , thinking there is going to be resistance, but there is none...he ends up hurling himself, with all of his momentum, face first into the floor. He couldn't get his hands down fast enough to break his fall. He messed up his grill real good...blood everywhere...It couldn't have happened to a better guy!

I've dislocated my knee in the gym and couldn't get it back in place...was taken out by EMS. I've also dislocated my shoulder during heavy db presses for shoulders.

i read a story where Lee Priest was doing benches with 455 and a guy who worked at the gym was showing a newbie around and the newbie rested his arm on one side of the plates that Lee was benching, he said the bar and plates went everywhere, maybe Lee can verify it.

I heard a story about a gym manager and the lead female trainer 69'ing in his office right before the gym was closing. The spick janitor came in the office to collect trash, scared the trainer half to death and she bit the manager's dick. He ended up needing 11 stitches.

Did see this one but was told about it. Happened at my university gym.
Little Olympic lifter guy doing the jerk part of the clean and jerk. Has his top weight (which is heavy, he eventually went to compete at the Commonwealth Games) on the bar not sure the exact weight. Gets two guys to help him bring the weight down and re-rack the weight on the stands. One guys starts day dreaming. Little lifter guy thinks he is going to help with weight. Weight gets dropped at one end, equipment goes flying everywhere. The guys who saw it were amazed no one got badly hurt.

Seen a man break his arm benching in a meet. Pretty nasty you could hear it snap from 100 feet away.
Seen a guy fall back during a squat attempt and the bar landed on the spotter and broke a bunch of ribs.
Watched a kid drop a Db on his face doing incline press...

i had a little accident myself some about 3 weeks ago. nothing major but.
did flat db presses pyramided up. forgot i put the previous dumbells on the floor. And when done with my heaviest set i dropped the dumbells down i managed to smash my finger between the db on the floor and the one in my hand. finger swoll up to about twice the size. still cant bend the finger :-[

Both of these were when i was an undergrad, there was a school bench press meet, and this dude had 330lbs on the bar and it was going up easy so his spotter wasnt ready, and it fell back right on his neck about midway up. He was squirming, but suprisingly didnt get hurt. Another time my buddy was squatting 600, and when he went to re rack the shit, the guy on the other side of me got the skin inbetween his thumb and finger pinched in the bars you rack it on. It took a second to realize what was making him scream, then we picked it up and his shit was bleeding everywhere.

MOS, i know what your talking about in a sense. in college i saw a kid doing a military press and his shoulder dislocated while pressing...fricken bar dropped on the back of his neck...he stood up and walked over to the desk sayin "ow..ow...ow" not one person helped him out...poor mug.

some overzealous dude was doing pullups and pulling a bit too hard. on his 5th or 6th rep he pulled too hard and actually fell on his ass
SAME GUY went over to do incline barbell presses wiht a 25 on one side and a 35 on other. My friend and i watched waiting for him to unrack it (we obviously didn't tell him) then he unracked it and the barbell did the seasaw thing and made a shitload of noise.

About 15 years ago at the college gym I trained at I saw a dude in the middle of training shoulders collapse while doing side laterals, pass out on the floor and puke his guts up all over the place. At the time my girlfriend (now wife) who was studying to be an RN basically checked all the guys vitals while an ambulance was called - the dude was in a totally unconscious state and convulsing a bit.
Turned out that he died in the hospital about three days later - he had suffered a major brain anyurism (sp???) and never regained consciousness. Could not have been any older than 21 years old either.

Only time i can remember coming close to an injury was doing standing calf raises.
I set the station to it's max (600lbs) after pyramiding up over 4 or 5 sets and i guess i'd been sweating quite a bit. So I start on the ground then put one foot on the platform, lift myself up, then put the other foot on the platform. After 2 or 3 reps my right foot fucking flies off the station because I had put it on a spot with some sweat on it and all the weight comes crashing down on my left leg.
I've never been so happy that i don't lock my knees on calf raises in my life. Nothing happened except for a loud crashing sound but if i had my knees locked I'm positive i would have shattered a knee cap. Now I always make sure the platform is dry.

I learned mine. Back in 03 after leg pressing i got up to load an extra plate or two and thinking it was on the sled safely (i was a bit woozy from the last set, breathing heavy) anyways it wasn't on there safely! it fell smack on my left toe. from 3.5-4 feet high. Hurt like a bitch and didnt feel like stopping. By the time i got to squats i realized my foot felt a bit hot, took off my shoe and saw red everywhere.i was bleeding like crazy, busted the cuticle (right where the nail meets the skin) and got a hairline fracture on my big toe. couldnt walk right for 2-3 weeks because any pressure on the toe was fuckin sharp pains, but i still did legs 5 days later, just stuck to extensions, and seated leg curls, etc...to this day i check and double check those plates on the leg press, and every exercise for that matter!
