Title: Question: What and where are heaven and hell?

Where do souls physically go when the body passes? Some people believe they will remain in the same body of consciousness and will be reunited with loved ones. But since no one has ever really gone to heaven or hell and back, (aside from the white light observations) how do people "know" where they are going?
