He was referring to bodybuilders as an example of how things looking good from the outside don't fix the real problems going on inside an organization. The reference was look at bodybuilders, they look great on the outside, after spending all their time in the gym and on roids, but inside they have a pair of grape nuts, and their thyroid is screaming 911.

So what, it's the truth, natural weightlifting is healthy, bodybuilding is not. Loading yourself up with 300 times the exogenous testosterone your own body naturally makes is not healthy, neither is taking thyroid stimulants or vet anabolics. Let's be realistic here.
