A family friend in her 50s has seen a slow swelling in her calf for the last few months. it's now 1/3 bigger than the other leg.
She's very reluctant to go to the doctor - we are all working to get her to go.
What could be possible causes? blood clot? something else?

She needs to go to the doctor immediately. Could be a heart issue....definitely a circulatory problem

Could be a DVT- deep vein thrombosis. Dangerous- she should seek medical assistance immediately.

Acute trombosis not likely because the process is too slow. Could be venous insufficiency, in that case the calf should be less swollen in the morning. If there's pain if the foot is flexed dorsally, go visit the doctor anyway, for it can still be a trombosis after all.
Heart failure is not likely because it's asymmetrical.

is it red , warm to the touch , does it hurt like an ache or bad stabbing pains ?
is she on any new meds lately ?
does she move around a lot or is she sedentary with it ?
if she is refusing treatment then I would give her the worst case scenario and tell her if she doesn't go she will most likely make things worse by waiting and may die .. is it a fear of Dr.'s or she thinks it's not anything that may be serious ?

One leg swelling over an extended period of time probably means inhibited drainage of lymph, for which one possibility at her age is cancer, spreading to lymph nodes. Venous insufficiency is another possiblity. Something, that definitely needs to be addressed to a doctor.
