I looks ike some inguinal hernia with the intestines stuck and twisted in the scrotum, so they got necrotic. They don't mention intestinal surgery or making a stoma, so I'm not sure...

I once had a patient like that, only without necrosis. With a stetoscope you could hear bowel movements in the scrotum. Was weird, but not that diffcult to fix.

This happened to me when I was little. I went through a phase when I would try and hold my poop. One day I looked down and noticed my sac was huge. Went to the Dr and he said my intestines were swollen and they were backing up into my scrotum. Now I shit everyday, sometimes twice a day.
