Both, really... but ultimately the guy firing the weapon for not checking his back blast area. But the dumbass that calmly walks behind someone ready to fire an RPG isnt very bright either....

This is another nasty one, for some reason these fellow never learn to stay clear of the back blast or to clear/check for barrel obstructions. You would think they'd have that situation in hand, since the RPG has been a favorite for 50 plus years, but....
