Anyways, I think guys like him get tattoos to make up for the lack of conditioning. Piana is the same, blowing up his arms and shoulders with synthetic filler and then put tattoos on it to make it look less fake.

I don't foresee him being totally honest in therapy and he needs long-term intensive therapy not sporadic sessions
