Not sure if this is shopped or not. It would take a large hand and small feet to be able to balance this girl. Either shopped or not, it certainly is a throwback to the classic b&w Weider/Draper/Arnold beach photos of the late 60's/early 70's.

Heidi Montag looking fit

It's definitely a cool shot, but when I tried to Google it's origins, one of the first hits was this shot of Sandow. Is that some sort of gas mask on his p3ecker? ???

I think it's a fig leaf, which was common at the time. Btw, I do not know the origin of the photo.

It's also common knowledge that all gas masks were made out of fig leaves during that era, so he's technically correct.

That's not Sarah.
Must be photoshopped.

the shadow would suggest it is not photoshopped, but I doubt he's balancing her. With today's cameras able to take many frames a second, it could have been a continuous motion up and down.

As expected, the symbol of bodybuilding emanates severe gayness and has a gut.

Yep, further research into the Victorian era has led me to several discoveries:
1. There is a LOT of hairy Victorian p0rn out there... and I mean a LOT...
2. There are certain overtones to a lot of Sandow's pictures that I hadn't noticed before...
3. He clearly shaves...
And yes, that is a fig leaf which was commonplace apparently, but how is he attaching it?!? Glue?

He's standing naked with a Fig leaf over his cock. Which overtones didn't you notice, the straight ones?
