More than likely he was injecting very frequently into the tris and ruptured/nicked a blood vessel causing a hematoma. This happens a lot when site injecting (Gregg Valintino had one but said it was caused by getting hit with a baseball bat)  He probably didn't get it treated the painful hematoma grows and gets infected after he keeps injecting into it.
