I get the context, but with Depp it's always hard to say what exactly is leading to his most recent metamorphosis.

The guy literally ages/morphs/mutates right before our eyes when prepping for roles. Much like Christian Bale, he is willing to go to unforeseen heights to deliver on his roles.

Now, if he did end up with AIDS, he would be the new face of it, possibly dethroning Magic Johnson.
