Are those fake weights? I just don't buy it...this happened to a 190-lb Russian powerlifter in 2012...and he died...
Also, I can see one hand failing...but both at the same time?

It's real. A while back a powerlifter dropped 1000lbs on his chest and survived.

I hate watching videos like this (not sure why I watch them). It makes me nervous when doing my own bench pressings with 25% of their weight.

LOL. I know you're joking with that number, but I think the risk of actual death may start at around 405-lb. I'm not exactly sure, but the human body can handle a certain weight dropping like that - just not over X pounds [the amount no doubt varying by person].
My lifetime max bench is 345, and that is just about the time that I'd start worrying...at 405, I would imagine a lot of men would be seriously injured or worse.
I just don't know if this video is real though...wouldn't that be enough to at least seriously hurt someone? Seems like he got away with not even a scratch.

I set it up on a burner phone at the McDonald's parking lot, so the IP address would be different. I think at the time, provincial IP addresses were used.
I think I outed myself, by saying I fear for everyone's health, using that account [which would have been obvious].
I'm a huge fan of Getbigger of the Past, Bast though.
He posted here for years. He's the real deal.

Ah...that makes since. But I think you are thinking 25% of 1,100-kg, which is 275-kg [551-lb]. That would make sense - as most can't do that. Although the bench shirt adds a lot.
You're 6'5" and 275-lb, aren't you, Hulk? So I imagine you could handle 275-lb pretty easily. Obviously most can't bench 275-kg, but the bench shirt adds a lot.
Did Mendelson ever hit 700-lb raw?
Here is the video I mentioned above...where the Russian powerlifter dropped 400-lb on himself in 2012, and later died.

I will say it's surprising we don't see more crushing type deaths given how much missed presses happen. A lot of it I think has to do with how fast your team can get you out from under and to the hospital, and if there is any bad rib damage. A few of the nastier bench press accidents I've read about over the years were where a broken rib stabbing into something really fouled things up.

The worst accident I’ve ever seen in a weight room is when I was at USC. I was actually there in the weight room the day that Stefon Johnson Drop the bar on his neck and caused him basically to lose his career I was brutal. And it was all due to the negligence of an assistant strength coach

That's horrible.
I didn't witness the accident below, but he was among the Ontario's Strongest Man competitors for several years, and this was the closest competitor that I knew who went through what could have been a horrific accident.
As it turned out - he was unscathed, save for a few scratches. I think he could have died...if he didn't fall where he did:
