Stallone must be also questioning his long term viability. Burt Young was 83, whereas Stallone is 77. I know that when you see your contemporaries pass away and they're close enough in age, it can make you tense up a bit.
This said while not knowing Paulie's past medical history and whether or not he was relatively healthy.
