Im guessing he's in an ER. The look on his face is one of concern, terror and denial. I can just almost imagine hearing him describe in detail his morning that day, before he suddenly felt a radiating pain in his chest, going light headed while taking a shit, and "the next thing I know Im in the back of an ambulance.....

Close he runs a test GH clinic I think the sign the back looks like that could be the place looks like he was going over pricing and somebody told him to go fuck himself and he’s like wait. Wait wait I’ll lower the cost.

I believe he invested his money that he earned during his 3 years with the WBF/Titan Sports (now known as World Wrestling Entertainment) into this clinic and some other things.
Shawn Ray, Troy's training partner, claims that the reason he turned down the WBF is because he was only offered $225K per year vs. the Zuke's getting $275K a year. And this was despite Ray placing 3rd at the 1990 Mr. O vs. Zuke's not having competed at all prior to the first WBF show.
