The right arm is in front of the light colored wall corner. Which throws off the outside of the tricep, making the arm look smaller. But, still the arm looks odd. Especially the forearm.

His skin is literally purple, why is it when you abuse steroids your skin turns purple? I'm guessing high blood pressure but it can't be the only thing.

To answer your question, what you're looking at is a man who realized he would never make a dent in the world of professional bodybuilding competition, so instead he is now working as hard as he can to promote his brand of supplements.
He delivers his snake oil with very effective packaging by making these YouTube videos letting customers know about the seedy underbelly of bodybuilding and the scams that other supplement companies sell forth as truth. He then, in between giving out "free" advice, makes a protein shake or two using his own line of products and/or gets on the treadmill, but not before taking a supplement of some sort (from his brand of course) to aid him throughout his workout.
He realized there is not a great future for the vast majority of professional bodybuilders and has instead dived aggressively into the one aspect of this industry that is most lucrative, that being supplements.
For what it's worth, at least he is entertaining (somewhat) and can put together coherent sentences.

Got to disagree with the "coherent sentences" part. I just clicked on his latest video at random intervals; here's a few quotes:

As far as todays youth goes (his target market) thats perfect pitch, they all talk exactly like that themselves.

My opinion is that once PMMA or a similar implant becomes safe (it's not now) those people who look up to Piana will rush to the nearest clinic to get theirs.
You are looking at the future indeed, this shit has the potential to be as big as breast implants are for women. Looks about as real too.
