Any way, getting back on topic, the guy's back looks like it has a tear on the right side, and his guts are sticking out or something. Some sort of hernia.

Even if it's all muscle, it still looks ugly and hurts the physique. I don't think anyone would want a back like that.

Sure as hell looks like it to me.  You can even see a site injection mark in that tumor on the bottom right side of his back.
