Years ago, I got an insurance form from a film company to fill out. It was rather unusual because all film personnel are automatically covered on film shoots. Occassionally risk assessment is done on the more major cast members and they are insured separately from the rest of the cast or production crew. Guys like Jackie Chan can't even get insured on their films. I figured this production company's insurer wanted more details to assess liability risks and to know what kind of premiums to charge the production company for insuring a completion bond on that episode. Anyway they started out asking innocuous quetions like age, sex, weight, etc., then asking questions like do you smoke? do you have any known allergies? ok fair enough... have you ever been hospitalized? if so what for, state all occassions and conditions., etc., hmmmm.... then they started getting into really personal questions like "Are you sexually active?" "Do you use birth control?" "What method of birth control do you use?", "When was your last menstrual cycle" "Have you ever had a venereal disease?" "Have you ever had an abortion?" I was like WTF?! I called my agent and told her I wasn't filling this damned thing out.
