A good buddy of mine is a mechanical engineer and we were talking about how this stuff, along with recalls and all that, can happen with all the checks and inspectors and etc. I kind of put it up to people talking to one another and just giving green lights to stuff based on what the guy before him said...but I think this is actually more probable.
