There were repeated outbreaks of plague, not just one. I think the first was in the 500s in Europe. Also heard it was actually pneumonic plague that claimed the most victims since it spread human to human and you couldn't necessarily tell if someone had it.Seems like new influenza strains from Asia are the most likely natural threat. Bioterrorism scares the shit out of me - some engineered bug in the hands of zealots who are convinced God will protect the righteous.
