Reasons for little progress in the gym

How come at every gym there is at least one guy who trains regularly at good enough intensity proper form etc but is still very normal looking as though he's not a lifter.

Not talking about fat permabulkers or super skinny guys. I assume its total shit diet, maybe genetics too, but it puzzles me why someone would spend so much time lifting for no results.

It's simply that they never eat over maintenance and get very little protein
