He has a torn biceps. Probably the long head at the shoulder.
Seems he has developped Osteoarthritis in both shouders, too.
He probably suffer from degenerative disc disease, osteoarthritis in the knees and hips, as well.

But, he is Arnold S. So he can afford the best physicians and the best treatments in the world.

I remember in Back to the Future where Marty went into 2035 and picked up that magazine at the 7-11 and you can clearly see Jay Cutler on the cover. Pretty darn cool.
