At one time HIV patients good get pretty big dosages of steroids and HGH legally. Many would keep what they needed and sell (or exchange
)the rest to Bodybuilders. Don't know if that's still the case
they certainly did it with GH a few years ago. ADAP (state's Aids Drug Assistance Programs) would pay for GH but not for AS to counter HIV wasting because it was considered an off-label use of AS. The drug companies had done lots of studies to show that huge doses of GH would add about 10 pounds of muscle mass. But with AS being generic, no company would spend the money to show their effectiveness. So states were paying thousands of dollars for GH, which had to be injected every day and caused joint pain at those doses, but wouldn't pay a few hundred for AS which was more effective, and could be administered completely in the clinic.
then on top of that, since ADAP is for people without insurance and limited income, much of the GH ended up on the black market to help pay the rent.
today, ADAPs still won't pay for AS, but they'll only pay for the GH in very specific cases, and usually require you to come in every day for the injections.
