The difference is supination. By tilting his wrists outward as he curls, he isolates the peak. Nothing but basic principles of physics at work here.

There was a proximal tear of the left bicep that is obvious from the October 2011 guest pose.

The issue is the filling of the gap with oil or whatever else.
