I'm guessing on the Brazilian accident, it wasn't firmly on safe, and the vibrations from people working out by it knocked it loose, or maybe someone brushed it.

I’ve used that type of Front Squat/Hack Squat machine hundreds of times... I’ve never had the urge to sit on the foot platform between sets.

I’ve used that type of Front Squat/Hack Squat machine hundreds of times... I’ve never had the urge to sit on the foot platform between sets. 

Once I  do any set,I stand up change weights,get some water, or just walk around and breathe....I never sit down between any sets.

Now I'm wondering about the squat machine in my basement. The catch seems really secure. Every time I'm on vacation I use a hammer strength squat machine. Makes me wonder if the safety catch is fully in position now.
