The guy had surgery on a broken neck. He literally has hardware in his neck and is now recovering. He is in great shape. There was nothing fake about his muscle but yeah break your neck so you can't train and you will lose some size.

If the muscle-mass cant be maintained without the boatload of drugs, its "fake water-balloon" muscle.
Once they go off, they piss out 70 lbs of retained agua...
