Plastic surgery is usually butchery performed by people with ZERO artistic ability. They're doctors who pass a lot of science courses but ironically they are unqualified for the very thing their job is about. Even 10 years of art school much less medical school school does not even close to give just anyone the high-end genetic art IQ needed to f*ck with people's faces, any more than 10 years of studying basketball would qualify 99.999% of people for the NBA.
