So this might be a bit dated because I'm 30 now, but I always heard the horse video as a reference to "Mr. Hands".
Basically it was a video of a dude getting fucked to death by a horse.

Last I read on it, the video is not what killed him. He released plenty of horse videos, and that was just another one of them. The one that he sustained internal injuries from wasn’t released.
