Visas are given by country when you enter. You just apply for them. Places like Australia and Canada have digital visas. They then stamp the passport when you enter.
Other countries simply stamp the Visa on your passport when you get to their borders. Some don't even bother.

Passport is the physical thing you carry. The Visa is the stamp they put on it basically.

Up until around 9/11/2001 you could travel to Canada or Mexico on a valid US state driver's license. No more. The last time I went to Canada, the border immigration was as stringent as entering the EU.
