1) Doesn't necessarily mean C7....vertebral prominens can be EITHER C7 or T1
2) The growth is most likely tissue that has formed OVER the area where the bar rests from squatting....thus protecting the spinous process. Think of the callous that forms under the distal head of the 5th metatarsal (i.e the outside bottom of your foot)
1)I don't want to get pedantic...but I think you'll find that
only C7 is named vertebral prominens, as in most specimens, it projects the farthest.. true T1 can have a equally long S.P, but its not termed vertebral prominens...just S.P. of T1....

2)I'm not too sure about the callous 'theory', as there seems to be no cutaneous signs of thickening (like as you said would occur on the base of a dropped metatarsal)...In my own experience I have seen patients who have never squatted with this appearance...its just and anatomical variation/anomaly..
