I was reading 240's story about a dead body and it reminded of when i used to work at a Paper Mill during Summer Break. This guy used to tell me to be careful, and he told me of a story of a young guy he used to work with.
It was young guy's first day, and he was on the ground directing some asshole in a crane, who had those huge metal bars (i don't know what they are called--support beams?), well the guy in the crane was drunk, and he dropped the support beams, basically on the guy. The man who was telling me this, ran over to the young guy to find him, he reached underneath one of the beams, and pulled his hand back real quick, he had a handful of the guy's brain. Guy was killed on impact.
Sad thing was not that is was this guy's first day, but he was newlywed as well. They fired the drunk that day. I'm guessing the wife got a settlement.
This is a extreme example of when a person at work f's up, it cost a co-worker his life.
