Bodybuilders, as opposed to overweight persons, have real breast tissue. To prevent recurrence (10% in the literature) of gynecomastia, most of the breast tissue should be removed. In some cases when not enough breast tissue is removed, recurrence is more likely. In many cases, this so-called recurrence is actually the lack of adequate excision.
So basically if recurrence happens, is actually due to error of the surgeon and not physiological causes.

The first thing the surgeon tells you during the consultation is that there is a chance it comes back. Clearly you don't know what your talking about. Rich Gaspari got it removed three time during his career. I went with my friend 2 weeks ago to get his Gyno remove( he is a national level competitor). The precedure went like this....
They put him out at 1 pm
at 2 liposuction was preformed with 2 incisions under his armpits for his entire pectoralis region.
at 3 they opened his nipples to removed the mammery glands and they closed him up; was released by 5 30 and how to stay in the nearest hotel for the following 24 hours.

My friend asked if he has seen the same bodybuilder twice before and he said there has been a few repeat cases and that has nothing to do with the surgery going wrong, but rather because bodybuilders are extreme with thier steroid intak. Ultimately you just have to pray that it doesn't comeback, but at worst it wont come back for several years so who cares.
