Wear and tear, straining. Basically when some of the contents of the abdomen protrude out through the abdominal wall. Only usually becomes a serious problem when they cause a lot of pain or the blood supply to the herniated contents is cut off and the tissue necroses. Then there's big problems

You can get a hernia at any time. I've had one- inguinal. I was born with a weakness in the abdominal wall, near the groin. Your testicles drop down through your abs, and sometimes the abs don't completely seal shut 100%, then it's only a matter of time before you discover you have one. I'd deadlifted, squatted and such, jumped off the bleachers at HS basketball game....no pain nor hernia. But one day I was pulling a weed from a flower bed at my job, felt dizzy and went home that night to find a small protruding bump a bit lower than my belly button. It was a hernia.
You can get a hernia from sneezing/coughing, twisting the wrong way....anything that puts pressure on your abs to any extent.
It's not just one possibility- but does just boil down to having a weaker abdominal wall than others. Then your intestines protrude through, causing the bump.
