Poser....you should have an IV hooked up during the night and not remove until after you shower.

Although 45 minutes won't kill you, you can do half a protein shake the moment you wake up if you're that concerned.  Then have your regular breakfast an hour later.

I typically do a full shake when I wake up and then my breakfast about 2 hours later, but I've started off the past couple of days with food and may try to stick with that for a while.

I think it depends on where you are at now.  Are you bulking or dieting?  Are you 8% body fat or 20%?  45 minutes really doesn't matter that much unless you are at a really low body fat trying to bulk without putting on fat or right before a contest.

shakes are usually easiest in the morning. its okay as long as long as you make sure to combine with your shake some vitaminmineral, some vegetable/fruit or veggie pill, and make sure your shake has the right amount &type of protein/carb/fat for whatever your current goal

45 minutes isn't going to kill you, if it was two or three hours i'd say down a shake.

About as relevant for your results as the type of underwear you wear.

That's way too long. You are totally eating away at your muscle-mass by not eating 40 minutes BEFORE you awake. Somebody suggested an IV drip of amino acids and glucose during the night, which is the best choice but if you can't do that then be sure to set the alarm for 60 minutes before you awake and eat 12oz of sirloin and 10 egg whites with 2 cups of oatmeal then go back to bed then when you wake up again be sure to have 60g of whey isolate to keep you in a positive nitrogen balance. Be sure that you are using your MuscleTech supplements because they are the most advanced muscle-building products in existence. Good luck!

No.

The good meal in the morning is to feast on your morning crap.

A good solid first-of-the-day dump takes a while to get out (coffee helps), and you don't want yer digestive system to be busy processing other food when you finally eat your poop with some sugar on.

Here = the routine: 1: Coffee immediately upon getting out of bed. 2: Wait around. 3: Shit. 4: Prepare the poop on a platter and add sugar. 5: Eat it. 6: Have a shower and rinse the remain of poop that the toilet paper could not catch out of your ass.
