My educated guess, the mother has Münchhausen by proxi. She feeds of the attention that her daughter is getting due to her weight, feeding her child is like feeding her own need for attention. She should receive a psychological examination, hopefully this will reveal grounds to take the child away from her care.
I wish we as a society could stand back and let things unfold naturally. All these bleeding heart liberals who want man out of nature can't sit back and let nature unfold right in front of them.

--let them reap what they sow
So when the parents are on their knees begging for insulin, but can't afford it because they've wasted all they have on pizza -- getting them into this position -- DO NOT bail them out.
Sounds harsh. But individuals have gotten so used to people saying:
"blame the parents"
"give them treatment"
"bail them out"
"poor kid" or "poor family"
...That no one is worried that if they fuck up they have to pay the piper.
Sure I feel for them, but the only thing screwed up with these people is that they have no real need to worry about consequences. Someone is going to give them whatever they want or need (attention, food, medical, whatever) and they will always be dependent slaves and we will always be paying to clean up their fuck ups.
I say we clone some saber tooth tigers...and release them into our cities.
