Something is not right - is this a hernia?

Something is not right.  Can anyone explain this?  Seen this in person?  This can't be a typical hernia.  What body part is this that is protruding?  What does this feel like? :(

Absorbed twin? Injection site---or, yes a hernia? Fux and Ruhl had one, but not that big....
