On a male, that's a sign that it was not sure how he was going to end up when one was a considerably developed fetus otherwise.
Also it indicates that he has low gh-levels and during puberty his test-levels didn't raise much, estrogen ended to dominate. Only thing that amazes me is, why he has no significant gynecomastia... as that's also a sign of messed up hormone balance... maybe he went to surgery?
