Theoretically, it seems like the more thoroughly you tear your muscle fibers up, (assuming you get enough recovery time) the bigger you should grow.

Why is it not that simple?

This is true for Lifetime Naturals.  The very best ones train nearly 7 days a week as much as possible.  Francisco Montealegre for instance, does chest about 4 times a week.

Well there's a balance obviously once they have been strained correctly you need them to heal, over training means your training muscles when they still require healing. They goal is to stretch the muscle in a manor that has the stretching occur down the entire length of the muscle.

To the general Public, there is no difference between Phil Heath and Francisco Montealegre other than that Montelegre looks a lot better.  I hope this helps.

Absolutely true, and before googling Montealegre, I couldn't have pointed either man out in a line up.
