It's just a plain metal instrument, probably warmed up a bit to help open the pore, you can order different sizes, just google "blackhead removal tool". The main difference is that they allow you to apply a lot more direct pressure to the area than just using your finger tips.

Also, these Indians are presenting really large ones, so you get a nice mess when you dig them out.
