Everyone has a cephalic vein, how fat are you? It's a big vein that comes out early, you needn't even be very lean to have it visible.
Not fat at all - my forearm veins are pronounced and so are the ones at pec-delt tie-in. I just don't have the prominent bicep vein - even after I've trained biceps and they're pumped it still doesn't show. Obviously you can see it, but it doesn't "bulge" out the skin like most bodybuilders bicep veins do. Just one of those things I guess - purely genetic.

Cephalic vein, means nothing, maybe a very small chance of 'Thoracic Outlet Syndrome" when veins, artery and nerves are pinched by ribs ans pectoral muscles, not unusual in professionals who keep their arms raised (painters etc...) or weightlifters. But, he has no arm swelling which would be a tell tale sign. I don not think Mr. Penn is either

Actually many don't have an upper arm Cephalic and some have one that is deep within the upper arm, means nothing either way.

 I thought you would know better - there have been many pro bodybuilders in contest shape who don't possess a prominent cephalic vein - Flex Wheeler and Jay Cutler to name a few - the vein has been visible, but not what you would call "prominent".
