It's just natural shut-down of some control systems of body because of too much alcohol.

The scrawny malnourished girl is covered in dirt and paint. She clearly hasn't eaten in weeks. They are sitting in a garbage filled shed and the guy in the picture has clearly pissed in his pants and he is just sitting there in a puddle of piss.
