I was in Brazil over the holidays and mostly spent my time at bars but did go to a club or two and yea they pack those mother fuckers in I mean its much more than shoulder to shoulder, you can barely fucking move in there.
And yea they give you a card like a hotel card/credit card that you use to get drinks on and it keeps your tab. Then when you go to leave you have to go through a process of going to the cashier, giving them your card and paying your tab.

The US usually has a big one every ten years or so. Most in old structures that haven't been brought up to code, also depending on the area, you use to see a lot of illegally locked exits, but they've really cracked down on that in the last couple of decades.
