This is what happens when you are obsessed with size and mass, and lose the majority of it.
You inject humongous amounts of oil to keep the illusion, but it's too evident, not fooling anyone.

Still, if it makes Derek feel better about himself, more power to him.

Illness, Age and excessive usage of Drugs has already set it's toll on the appearance. Very difficult to recover.

dudes face is a mess... can't tell if it's from his kidneys not flushing water, or maybe he went off all or most his gear and he's putting a ton of fat on his face..... i'm guessing it's a combo of both.

I only hope that if I ever get to the point where my kidney's are failing (or heart or liver or whatever), that I have the wisdom to pull the plug and move on like Tom Prince, and not continue to chase 'the dream' like DA.

perhaps DA actually went off all the AAS, and the doc's have him on boatloads of prednisone and what we're seeing is the result of that..... but I seriously doubt it. also seems to have more oil in his arms... either that, or him losing mass just makes the past SEO use is just more apparent.

if he lost 20-25lbs of muscle overnight, it would give his face an emaciated look but that's independent of the continuing lube jobs.
