I bought some Yurbuds earphones with patented twistlock technology 'guaranteed not to fall out' - they fall out.

I've had good luck with christmas tree style rubbers pictured below.  I cut off the smallest one on the end. 

I wear an ipod mini (matchbox size) clipped to my collar so there's not much length of wire swinging around, and I run the wire from the back of my head up over the top of my ears.  Good luck finding a set with a short wire tho.
