What cause this side effect

I see a great bodybuilder who has a big muscle gut.  I understand it is caused by hgh.  On top of his belly, a little small round stuff push out.  I wild guest it is side effect of roid or hgh or ....  why he causes this side effect?  is it really bad?

hernia. nothing to do with performance enhancing drugs, but heavy pressure on the abdomen (by lifting, eating tons)
its broken abs and a broken belly button basically.
