He was competing whilst on dialysis, he used to cover the swollen veins they use for dialysis with training aparel as you can see on his firearm. I don't think he competed much post transplant, these days he looks normal off the juice albeit still grains and has some size for his age.

Two bodybuilders, tonza muscle, zero kidney function....thanks to medical advancement they're doing great today. 30yrs ago people with kidney failure never used to last.
