I have a theory that is likely full of shit, but it seems guys who go into rehab and stuff for booze and drugs and get into shape always seem like they're using some stuff.  Maybe they're not afraid to use a drug that's "safer" compared to what they were in rehab for.

Those actors you talk of are the ones that got superhero movie roles.  Efron has never had to work out for any role.  Seems he just chose to be in shape.  Most of his movies he's only ever taken his shirt off because he is in shape but it wasn't the purpose of the role.  Seems the new Baywatch movie is the first time he was required to be in shape so he probably got jacked a little extra for it.
